INTAKE FORM
This form asks for both your “First Name” and “Real First Name (if different from legal)”. If those names are different, trust that this field and the signature at the end of this form are the only times you will have to provide your legal first name. I hate that I have to ask for it, because you know who you are better than anyone else. You will only ever be referred to by your real first name. If you’re trying out new first names, and you want your answer for your “real first name” to be changed at any point, I’ll be glad to change it. Nobody is getting deadnamed on my watch!
